Registration

Welcome to the 2011-2012 Season!

Registration process:

1. Register your player(s) with the USA/MN Hockey website and print confirmation page

2. Fill out and print Consent to Treat form (available on the Forms page)

4. Attend Registration and bring:

  • USA/MN Hockey Confirmation
  • Consent to treat form
  • Birth Certificate
  • Payment
  • Your Player(s)

For USA Hockey online registration:

1. Go to the USA Hockey Website.
2. Choose member type Ice Player/Coach.
3. You must be 18 years of age to process a registration. Check the box that acknowledges that YOU are 18 or older.
4. Select the correct season:  2011-12.
5. Follow the prompts…fill in all data correctly.
6. Process payment – Visa, Master Card, Discover or American Express (credit or debit card). A Visa gift card will also work.
7. You may print your Confirmation page at this time. It will also be emailed to you immediately after the registration is completed. Forward this email to registration@jchockey.com

Each participant will pay the USA Hockey fee of $30. There is an additional Affilitation fee of $10 that registers your player for MN Hockey.  The USA/MN Hockey fee is waived for age 6 (birth year 2005) and younger.  The registration must still be completed to get the confirmation number.

Registration Location, Dates, & Times:

Registration will be held at Phalen Rec Center (1000 Wheelock Pkwy) Check dates below:

Squirts/10U and above September 18th, 2011

  • Squirts/10U Girls 4:00 – 5:30 pm
  • Peewees/12 Girls 6:00 – 7:30 pm
  • Bantams 8:00 – 9:00 pm

*An additional registration will be held on September 25th from 4:00 – 6:00 pm

Mites & 8U September 25th, 2011

  • Returning families 5:00 – 7:00 pm

New Mite Registration October 2nd, 2011

  • 3:00 – 6:00 pm

*Parents should bring players to registration to be measured and if necessary, bring in equipment that doesn’t fit.

Please note-  The first installment of your player’s fee will be due at registration with the remainder due after teams have been selected.
